The Role
We are seeking a compassionate and motivated Case Manager to join our ASSIST program, providing outreach support to individuals aged 18 and over who are experiencing, or at risk of, homelessness.
This role operates within a case management framework, focusing on early intervention, collaborative engagement, service coordination, education, referral, and advocacy to help reduce homelessness in our community.
The ASSIST program delivers mobile outreach services across the ACT, meeting clients where they are and working alongside them to access the support they need to their housing goals.

About Us

About You
The ideal person we are looking for has:
Relevant qualifications and/ or experience in working with children and young people.
Demonstrated experience working with children, young people, and their families/ kinship/carers in frontline service delivery.
Knowledge and understanding of the key issues experienced by children, young people families/ kinship/carers engaging with harmful/risk taking behaviours.
Engagement with children, young people, and families/ kinship/carers through early intervention strategies and self-determination
Develop, and support relationships with the community service sector to support referrals for children, young people, and their families/ kinship/carers.
Hold a current drivers’ licence, a current Working with Vulnerable People card and be willing to undergo a police check.
Successful applicants must complete a National Police Check.
You must also hold:
· A current driver’s licence, and
· A Working with Vulnerable People Card (ACT) and/or NSW Working with Children Check
