The Exciting Opportunity:

On a day to day basis you will be responsible for:

Conducting regular health assessments & tracking disease progression and response to treatment.

Developing care plans coordinating care with other healthcare professionals.

Anticipate patients changing health needs and manage requirements appropriately.

Provided support to patient s and their families by responding to queries and feedback in liaison.

Maintaining clinical documentation to a high standard in a timely manner including referrals.

Administering medications and treatments as prescribed.

Participating in or leading chronic disease management programs.

The Successful Candidate:

Experience in Chronic Disease space OR post grad in chronic disease
Bachelor of Nursing / AHPRA registered
Full working rights within Australia

Open for an adventure to and working in a regional setting

Full Australian working rights and able to pass a national police clearance

Highly organised, work well under pressure and have strong time management

Relocation and accommodation full or partly covered depending on location
