About the Role:

We are seeking an experienced and compassionate Family Care Coordinator (Case Manager) to join our growing team.
You will manage a caseload of participants and their support teams, providing leadership, mentoring, and holistic support to foster the best possible outcomes.

Key Responsibilities:

Conduct assessments and develop personalised support and risk management plans.

Mentor and guide support workers to enhance service delivery and participant outcomes.

Lead incident responses and support teams during critical events.

Collaborate closely with families, allied health professionals, and external stakeholders.

Maintain accurate and compliant documentation.

Assist with community engagement, marketing, and service growth initiatives.

About You:

Strong understanding of the NDIS, NDIS funding, rostering and various aspects of disability support services.

Excellent communication, organisational, and relationship-building skills.

Knowledge of Autism Spectrum Disorder

Prior experience in support work, case management, essential.

A proactive, solutions-focused approach and genuine passion for empowering families.
