• Sound Aged Care and Home Care Knowledge with previous Case Management experience
• Highly developed communication and relationship management skills
• Exceptional problem-solving skills and ability to relate with those navigating the Aged Care system

• Some who has a network within your local area to tap into and build up on the current client list

What your days will look like;

• Managing and overseeing a portfolio of Home Care Packages clients
• Working with the rostering team to schedule the most appropriate carers for your clients
• Collaborating with the Frontline Team Leader to ensure the carers are equipped to service your clients in the best possible way
• Managing issues, mitigating risks, and overall providing exceptional care to your clients
• Identifying potential risks and implementing processes to enhance safety and mitigate them
• Attending to sign ups, conducting assessments
• Building your local network to build your client portfolio

What you will need to be successful;

• Current driver’s license and reliable vehicle
• COVID vaccination & booster
