In this Permanent Full-Time role as a Case Manager based at our Lakemba office, you'll play a vital role in delivering person-centred care coordination to our clients. Your responsibilities will include attending consultation visits, conducting intake assessments, developing care plans, and managing and monitoring client wellbeing. Your advocacy for your clients' care needs and building strong relationships will be at the core of this role.
About the Role:
As the Aged Care Case Manager, you will be integral in the launch and initial operations with responsibilities including:
• Initial client assessment, planning and agreements.
• Develop, implement, evaluate, and regularly review client-cantered care plans
• Work closely with roster team to coordinate the delivery of quality care
• Oversee the financial management of care planning to ensure services are delivered according to legislation and within allocated budget
• Maintain up to date client records of assessments, care plans and services
• Be involved with client changes and incident reports
• Providing advice and expertise on ways to improve services and q
