This is a Fixed Term Full Time appointment until 16/06/2025, with the possibility of extension(s).

Position Profile: In consultation with the Manager Avon & Central Primary Health develops, coordinates, plans, implements and evaluates social work services and promotes an interagency holistic approach to care of families and individuals with complex social issues in the communities serviced by the Population Health Avon & Central.
