About the Role:
- Provide comprehensive bio-psychosocial assessments, individual and family counselling, psycho-education, and support for adaptation to medical and lifestyle changes.
- Conduct clinical assessments, offer advocacy, referrals, counselling, and crisis intervention, ensuring patient-centred care.
- Collaborate with multidisciplinary teams and external agencies, ensuring efficient discharge planning and patient flow.
- Contribute high-level clinical expertise and lead education initiatives for staff and patients.
- Lead, initiate, and participate in research and quality improvement projects, fostering continuous professional development.
- Supervise staff and students, and support excellence in clinical practice through evidence-based training.
- Participate in the on-call social work crisis roster, ensuring responsive and timely care in emergencies.
About You:
- Alignment with Mission and Values: Demonstrated commitment to the Philosophy, Mission, and Vlues of Mary Aikenhead Ministries and St Vincent's Health Australia.
- Qualifications and Experience: Degree in Social Work (AASW Eligible) and evidence of clinical experience practising Social Worker with inpatients and outpatients with a cancer diagnosis or terminal illness.
- Specialised Expertise: Skilled in bio-psychosocial assessment, counselling, therapy interventions, case management, advocacy and discharge planning (including NDIS/ACAT/RACF placements etc).
- Crisis and Complex Care Competencies: Expertise in crisis intervention, solution-focused approaches, motivational interviewing, and complex assessment in an acute health setting.
- Leadership and Teamwork: Ability to contribute to research, education, and quality improvement, and provide clinical supervision, with strong communication and negotiation skills.
