Senior Social Worker
We are looking for a Senior Social Worker superstar to take over the Home Care Director’s established workload and clients so she can take on other duties. You will have extensive Social Work experience which will be required for this busy position. The person that we are looking for will have a ‘can do’ approach and will be tenacious when advocating for our clients.
You will have an in depth understanding of the NDIS space and have had experience providing support coordination to complex clients. You will be proactive in meeting the needs of our clients and anticipating the changes in health status and the need for reviews and assessments. You will have a working knowledge and understanding of physical health changes that come with chronic health conditions and will be able to pre empt what is required for the client and their families. The position will offer a variety of support coordination and case management.
The position will mentor our new graduate Social Worker and your management experience will have you contributing to policy and procedure development, continuous improvements and daily operational activities. The position is not based on billable hours.

To be successful in the role you:
Significant experience as a Social Worker (5+ years)
Degree level qualification as a Social Worker and registered with AASW
Experience working with and coordinating support for people with complex disabilities with high intensity care requirements.
Excellent understanding of the NDIS and experience working in this space!
A large part of the role supporting and communicating directly with participants, their families and other support networks to provide information and understanding of their NDIS plan
Ability to work collaboratively with a multidisciplinary team approach
Build effective relationships with external stakeholders
Blue card and NDIS Workers Screening Clearance or willingness to obtain
Current Australian Driver’s Licence
Be available to participate in the On Call roster, once every 8-10 weeks (and paid a generous on call allowance)
Some travel will be involved in the position as we service clients from north Brisbane to Northern NSW and you undertake home visits when onboarding clients or undertaking client catch ups. We have company cars available for when you are out and about!
