About the role

The Place For Belonging is seeking an energetic and vibrant Support Coordinator to join our team in Caboolture, working across the Moreton Bay and sunshine Coast regions. You will join a fast-growing NDIS-registered organisation, committed to empowering individuals with disabilities by delivering personalised, high-quality support coordination services.

In this vital role, you will work collaboratively with NDIS participants to navigate the NDIS system, connect with appropriate service providers, and achieve meaningful goals. By engaging with NDIS providers, mainstream services, government departments, and private health professionals, you will coordinate a wide range of services including allied health, personal care, community access, employment support and housing.

The end goal is to create an environment that helps individuals achieve everything from the little things in their daily life to the wonderous moments that inspire and help them realise their potential. We are authentic in supporting our staff and helping our participants shift from merely existing to living a full life and accountable for making a difference in their lives and our community

What you'll be doing

Work with NDIS participants, families, and other stakeholders to implement, monitor and review NDIS plans

Support implementation of all supports in the plan, including informal, mainstream and community, as well as funded supports

Connect participants with both formal and informal supports to improve their quality of life

Identify, refer to, and coordinate appropriate service providers and supports for participants, including coordinating the implementation of service agreements

Modelling an ethical, customer centred practice to provide support to the participants to achieve their goals;

Provide ongoing support and guidance to participants and their support network to ensure that goals are achieved and services are delivered in accordance with the participant's allocated NDIS plan and budget.

Complete and submit support coordination reports to the NDIA, including implementation reports, progress reports, and end of plan reports.

You will have:

A positive and proactive attitude with high energy levels and the ability to authentically engage with Participants, tempered with strength and adaptability to deal calmly with challenging situations and complex behaviours with a rational approach

Effective time management and organisational skills, ability to prioritise work and meet deadlines

Demonstrated experience in working with people with a disability and mental health issues

Demonstrated ability to thrive in a fast-paced environment and handle multiple tasks with ease

The ability to communicate effectively with a diverse range of people to build trust and develop rapport

Demonstrated ability to liaise with a wide range of stakeholders including other service providers, family members, advocates, and government agencies

The ability to work both individually and as part of a team to support Participants achieve their goals

Exceptional listening and communication skills, combined with the ability to think creatively and use initiative

Excellent verbal and written communication skills is a must in maintaining reports and case notes

A genuine passion for making a difference in the lives of people with disabilities

An ability to assess and respond to individual needs
