About the role
As a result of significant growth, we're seeking dedicated and experienced Support Coordinators to join our team in Eight Mile Plains. This is a full-time position with a hybrid work model.
As a Support Coordinator you will work closely with clients to effectively utilise their NDIS-funded supports, connect them with suitable service providers and allied health professionals, and monitor their progress towards achieving their plan goals.
Responsibilities include:
- Support clients and families in accessing and coordinating services.
- Build relationships with internal and external stakeholders.
- Implement supports outlined in NDIS plans.
- Help clients develop skills to coordinate support and engage in the community.
- Empower clients to understand and implement their plans.
- Be responsive to client needs, especially in crises.
- Conduct regular progress reviews and report to the NDIS.
Skills and Experience:
- Understanding or knowledge of the NDIS
- Experience in support coordination or similar
- Familiarity with psychosocial disabilities (advantage)
- Excellent interpersonal skills and the ability to collaborate effectively
- Strong written communication skills
- Demonstrable problem-solving skills
Requirements
The successful applicant will have or be willing to obtain
- Driver’s Licence
- QLD Blue card
- NDIS Worker Screening Clearance (yellow card)
- Relevant qualifications in disability services, social work, or related fields (desirable)
