About the organisation:

Russian Ethnic Representative Council of Victoria (RERC/Russian Aged Care), a charity founded in 1984, is the leading community-services organisation representing the interests of Russian-speaking people living in Victoria. RERC provides a range of services state and federally-funded to support elderly people, people with disabilities and disadvantaged people with CALD background. Our current programs include the Home Care Packages Program (HCP), Commonwealth Home Support Program (CHSP), Aged Care Volunteer Visitors Scheme (ACVVS), brokerage services in aged care, CALD programs and various community awareness programs. RERC also acts as an umbrella for a range of community organisations to provide a platform for various cultural initiatives and community events.

The role:

Reporting to the Team Leader, Support at Home, this newly created role has the dual function of case managing Home Care Package clients as well as taking the role of practice lead, being the first point of contact for escalations and acting as second-in-charge for the Team Leader, Support at Home.

Among other things, the Senior Care Advisor will

· Maintain a case load of Home Care Package clients and oversee the whole life cycle of a HCP, including intake, onboarding, risk and needs assessments, development of goal-directed care plans, coordination of services and supports, referrals, regular reviews and discharge.

· Act as second-in-charge for the Team Leader, Support at Home, including backfilling the Team Leader during planned and unplanned absences.

· Act as practice lead for a team of Care Advisors by providing training and advice, coaching and mentoring, secondary consults and upskilling, as directed by the Team Leader.

· Act as initial point of contact for client incidents and feedback escalations for HCP clients, as directed by the Team Leader.

· Support the Team Leader, Support at Home, to proactively develop new leads and manage the HCP pipeline in order to grow the HCP program in line with organisational plans.

· Assist with quality improvement initiatives, such as file audits and self-assessments.

· Support the Team Leader, Support at Home, with other senior duties as directed and agreed upon.

Skills and experience:

To be successful in this role, you will have

· Certificates or tertiary qualifications in a relevant field (Social Work, Disability, Aged Care, Community Services, Nursing, Allied Health, Human Services etc.) desirable.

· Demonstrated extensive experience in the delivery of Home Care Packages as a Case Manager, Care Advisor or similar.

· Strong understanding of Home Care Package guidelines and regulations and all practice-aspects related to the delivery of HCPs, such as assessment, goal planning, service delivery and coordination, referrals etc.

· Experience in role of practice lead/Senior Care Advisor or demonstrated informal leadership skills.

· Understanding of the Support at Home reforms.

· Good financial and business acumen and ability to think logically and analytically.

· Well-developed interpersonal skills with an ability to develop and maintain positive working relationships within a team as well as broad range of stakeholders.

· High degree of initiative, ability to work autonomously and with strong solution-focus.

· Strong drive for continuous improvement and innovation.

· Intermediate computer skills across the range of Microsoft Office suite including Word, Excel, Outlook, and Power Point

· Fluent Russian highly desirable.
