About the role:

The overall purpose of the Client Liaison is to engage in client relationship management and packaged care case management with clients, their carer/s, family and advocates for home care and disability services. The key requirement is effective working relationships with key personnel including Concierge, Multidisciplinary Team Coordinator and Service Coordinator.

The Client Liaison role may undertake assessment, support planning, packaged care coordination and case management activities designed in collaboration with the client to assist the client to achieve their identified goals. In addition, this role supports the client to manage and monitor their budget as appropriate to ensure care packages are financially sustainable, and monitors the services provided to ensure these are in accordance with relevant service models and guidelines.

This is a full-time permanent position for HAC South Coast - Robina being the main office. Travel around this area to clients home is required in this role.

Key Responsibilities:

  • Build and maintain positive client relationships as the primary contact for service delivery.
  • Conduct home visits to assess client needs and support goal setting.
  • Collaborate with key personnel to ensure effective service delivery.
  • Manage client budgets for sustainable care packages.
  • Implement and review care agreements in line with Consumer Directed Care (CDC) principles.
  • Provide advocacy, support planning, and tailored case management.
  • Maintain ongoing communication with clients and families to meet care needs.

Key Requirements:

  • Current National Police Certificate
  • Current Drivers Licence and own reliable vehicle
  • Dependent on Service based staffing models, this role is required to possess:

o Minimum AQF Level 4 qualification in ageing support, disability or equivalent and;

o Attainment of nationally recognised competency in case management or equivalent or willingness to attain

  • Certificate IV Ageing, Cert IV in Lifestyle & Health, Cert IV in Disability or equivalent
  • Experience with Microsoft Office Suite; familiarity with consumer databases is a plus
