Duration: Full time, fixed term until 30 June 2026
Base Salary: $86,905 to $95,515 per annum (Admin 5.1 to 5.4)

This position is identified for an Aboriginal and/or Torres Strait Islander person and is intended to constitute a special measure under section 8 (1) of the Racial Discrimination Act 1975 (Cth), and section 57 of the Anti-Discrimination Act 1996 (NT).
Play a vital role in supporting Aboriginal and Torres Strait Islander peoples mental, emotional, and cultural well-being in the Greater Darwin Region.
Who are we?
Established in 1991, Danila Dilba Health Service (DDHS) is an Aboriginal community-controlled organisation committed to providing culturally appropriate, comprehensive primary health care services and community development within the Biluru (Aboriginal) community of the Yilli Rreung (Greater Darwin) Region in the Northern Territory.
We work to address the social determinants of health including the broader issues that affect our people's health, through evidence-based research and public advocacy. We also work to empower individuals, families, and communities to optimise their health.
About the role
The SEWB Care Coordinator provide culturally safe and trauma-informed support to people at risk of homelessness in collaboration with external services and health care providers.
Responsibilities:
- Provide high-quality services to Aboriginal and Torres Strait Islander adults who have been referred and accepted into Salvation Army's Supported Housing Program.
- Initiate, coordinate, and support the planning, development, and implementation of culturally appropriate interventions, including client support activities, and develop individually tailored SEWB care plans.
- Provide short to medium-term case coordination support. Check-ins as required to support people to remain linked with appropriate support services to facilitate ongoing care.
- Monitor and assess clients' needs to implement up to date changes to care plans to ensure appropriate support is in place.
- Other duties as required, consistent with skills and experience, as directed by the supervisor.
To be successful you will need:
Essential
- Identify as Aboriginal or Torres Strait Islander
- Cert IV Mental Health/Cert IV in Aboriginal and/or Torres Strait Islander Primary Health Care or equivalent training/ work experience
- 2 years of experience in similar field
- Knowledge of the social determinants of health (environment, economic and social inequalities) that have a profound impact on the health and well-being of Aboriginal and Torres Strait Islander people.
- Demonstrated ability to use culturally sensitive ways to ensure improved client access to DDHS and/or other health services.
- Proven teamwork, communication, and interpersonal skills with the ability to constructively negotiate outcomes with a diverse range of people and service providers.
- High level of personal resilience and flexibility with the ability to deal with challenging situations.
- Demonstrated computer skills, especially in the use of client information systems or similar.
Desirable
- Experience working in an Aboriginal Community Controlled Organisation (ACCO)
- A tertiary qualification in Mental Health Nursing or Psychology along with a current registration with AHPRA.
- Demonstrated experience in mental health and social and emotional well-being assessment, treatment and rehabilitation methods appropriate for Aboriginal and Torres Strait Islander people.
What we offer:
- 6 weeks annual leave per annum
- 5 days paid leave during our end of year shut down period (in addition to annual leave).
- Salary packaging of up to $15,990 per annum.
