The Role

As our team continues to grow, we’re looking for experienced High Intensity Support Workers to

provide specialised care to both adults and children with complex needs, delivered in their homes

and communities. At Evolving Care, our Support Workers deliver high-quality, person-centred

support that promotes each participant’s safety, well-being, and independence.

Key Responsibilities

 Follow detailed care plans within your scope of practice

 Provide high-level supports, including:

o PEG enteral feeding

o Seizure management

o Wound care & pressure area care

o Diabetes management

o Complex bowel care

o IDC/SPC catheter care

o Tracheostomy care (if qualified)

 Assist with daily routines and home maintenance

 Maintain clear and accurate case notes

 Implement and Follow Behaviour Support Plans where applicable

Essential Requirements

 Experience working with complex health needs such as PEG feeding, Seizure management,

Wound care management, Complex Bowel Care, Hoisting, Medication management

 Certificate III in Individual Support or Certificate IV in Disability Support, or Certificate III or IV

in Mental Health or AHPRA-registered RN/EN

 Strong clinical decision-making and communication skills

 Confident working independently and managing your time

 Current NDIS Worker Screening Check and Blue Card (or willing to obtain)

 Current CPR & First Aid Certification

 Police Check

 NDIS Orientation Module Certificate

 Australian Driver’s Licence, registered & insured vehicle
