About the Role:

The Intake Social Worker is the designated point of contact for all potential clients and or representatives providing a responsive service to move them through the pre-engagement process to service delivery. The position is responsible for the implementation of a coordinated intake process that identifies client’s support needs, determines eligibility for services and the collation of client specific data.

This position will support an Integrated Health Service model to increase access to programs and services to achieve health and wellbeing outcomes for the veteran community. This will include assessing and coordinating incoming and outgoing referrals within relevant programs and services.

Key Selection Criteria:

• Demonstrated ability to provide high quality, efficient, client centred and evidence based social work assessments.
• Demonstrated ability to use high quality verbal and written communication to work co-operatively within the ecosystem to achieve the best patient centred outcomes.
• Demonstrated ability to apply clinical practice knowledge and expertise to effectively provide risk assessment and triage services to the Veteran community.
• Demonstrated ability or ability to rapidly acquire knowledge and skills to provide Veteran led care through a telehealth practice model.

About You:

• Relevant degree /Masters in Social Work
• Australian Citizen /Permanent Resident
• Current registration within AASW
• Preferably 1-2 years experience in a previous Social Work role.
• Experience working within a risk assessment environment or roles focused on rapport building, education, advocacy, and support is highly desirable.
