About the role:

The Intake Social Worker is the designated point of contact for all potential clients and or representatives providing a responsive service to move them through the pre-engagement process to service delivery. The position is responsible for the implementation of a coordinated intake process that identifies client’s support needs, determines eligibility for services and the collation of client specific data. This will include assessing and coordinating incoming and outgoing referrals within relevant programs and services.

Responsibilities:

Provide high quality, efficient, client centred, and evidence based social work assessments.

Communicate effectively within the ecosystem to achieve the best patient centred outcomes.

Apply clinical knowledge to effectively provide risk assessment and triage services to the Veteran community.

Rapidly acquire knowledge and skills to provide Veteran led care through a telehealth practice model.

Requirements:

Relevant degree qualification

Current registration within AASW

Minimum 1 years of professional experience as a Social Worker. New grads welcome to apply

Focused on rapport building, education, advocacy, and support is highly desirable.
