About The Role
The Community Nursing role will be working closely with the Enrolled Nurses and Disability/Aged Care Support Workers to provide and facilitate safe, responsive nursing care for our clients living with disabilities in their homes and other community-based settings. This role will involve evaluating and implementing clinical care plans, assessing, and training Aged Care/Disability Support Workers on various high intensity complex care support activities.
You will be employed on a full-time role and will have access to a company car if required.
Community Nursing Duties May Include, But Are Not Limited to:
In-home Clinical assessments
Providing independent nursing interventions, treatments, and therapies such as medications, and monitoring responses to treatment and care plan
Providing health education to clients and their families and assisting in planning and implementation of health promotion activities
Working with complex clients to manage bowel care, enteral feeding, tracheostomy care, urinary catheters, ventilation, subcutaneous injections, incontinence, Diabetes, seizures, wound management, etc.
Mealtime preparation and Delivery
Reporting to the NDIS on disability-related health needs of a participant through the portal and using established NDIA processes
Essential For The Position:
At least 1 year experience in working with patients under the requirements of the NDIS
At least 2 years of experience in providing support and care for patients with disabilities
Current AHPRA registration as a RN Div 1
Excellent communication and cultural sensitivity
Valid Australian Work rights, Employee Working with Children Check, current National Police Check and First Aid & CPR certifications; and
Working knowledge of digital rostering & CRM systems
