About the role
This part-time/full-time Support Coordinator role at HEALTH AND COMMUNITY SERVICES in Thomastown, Victoria. This is an excellent opportunity to make a meaningful impact in the aged and disability support sector. As a Support Coordinator, you will play a crucial role in empowering individuals to achieve their goals and live independently within the community.
What you'll be doing
Collaborating with participants to develop, implement and monitor individualised support plans
Coordinating and connecting participants with appropriate service providers to address their needs
Advocating for participants to ensure they receive the necessary support and resources
Monitoring the progress of participants' goals and adjusting support plans as needed
Maintaining accurate records and reporting on participants outcomes
Participating in team meetings and professional development activities
What we're looking for
Relevant tertiary qualification in a human services field, such as social work, psychology or disability studies
Demonstrated experience in a similar support coordination or case management role
Strong understanding of the disability support sector, including relevant legislation and funding models
Excellent communication and interpersonal skills, with the ability to build trust and rapport with participants
Problem-solving skills and the ability to think creatively to find solutions
Strong organisational and time management skills
Commitment to providing high-quality, person-centred support
KPIs
All approved support coordination hours are billed in accordance with the individuals NDIS support plans.
All hours of support coordination provided are monitored, recorded, and accounted for
NDIS participants service agreements in place for all Support Coordination participants
A range of options is provided to participants to select from to achieve the outcomes in their NDIS support plan
Ensure all goals identified in the participants NDIS plan are implemented, where practicable
All Participant NDIS support plans are reviewed as required

Our ideal candidate has:
Relevant tertiary qualifications in disability or a related discipline
Experience within disability services or related services
Demonstrated service knowledge of Disability and Mental Health.
Demonstrated methods that promote engaging individuals with a disability with their community and a commitment to principles involved in community inclusion;
Demonstrated time management in fast-paced, complex service environment;
Ability to use all relevant technology including tools enabling mobility and work autonomously.
