EPIC Support Services assists individuals living with significant and complex Brain Disorders (Mental Illness, Acquired Brain Injury, Autism, Behavioural and Neurodegenerative Disorders) to lead meaningful lives. EPIC is dedicated to working with vulnerable people and making sure that they don’t slip through the cracks.
We currently have a Part-Time vacancy 3 days per week for a NDIS Support Coordinator to join our Melbourne team based in St Kilda, supporting complex participants to achieve their NDIS plan goals. This position has the ability to increase to full-time.
About you and the role:
The Support Coordinator will support a caseload of complex NDIS participants to remove barriers, build capacity and strength through respectful relationships, plus ongoing skilled support to assist them to live independent, contributing and meaningful lives in their chosen community. By working collaboratively with participants, families, carers and the broader service system to identify, plan, design and coordinate NDIS supports, enabling them to take control and overcome barriers to better manage complex challenges of day-to-day living.
About You
Demonstrated experience working with people with brain disorders that include Mental Illness, Acquired Brain Injury, Autism, Behavioural and Neurodegenerative Disorders.
Tertiary qualified in Mental Health, Psychology, Counselling, Social Work or relevant to the field of human services or allied health
Demonstrated knowledge of NDIS plans
Demonstrated experience with AOD, Justice issues, homelessness and crisis intervention work.
Tasks & responsibilities
Provide intensive person centred support for participants of the NDIS to link them into required supports in accordance with their NDIS plan goals and funding.
Visit participants in their homes
Linking participants to support providers
Complete plan goals progress reports to the NDIS
Networking and collaborating with community and government agencies
Ability to liaise with care teams, informal and formal supports and legal services
Well-developed interpersonal and communication skills, both written and verbal
Strong organisational and administrative skills
