About the Role – Make a Difference as a Support Coordinator
As a Support Coordinator with DSP, you will work closely with NDIS participants to coordinate the services and supports they need to achieve their personal goals and build greater independence. You’ll be the key liaison between participants, service providers, and the NDIS, ensuring everything works together smoothly. This role offers autonomy and flexibility – you can set your own schedule and work independently, all while being backed by a supportive teamthat’s always ready to help.
In this role, you will:
Assist participants in understanding their NDIS plans and confidently navigate the system.
Connect individuals with the right service providers and community resources to meet their goals.
Collaborate with participants (and their families/carers) to develop personalized support plans that reflect their needs and aspirations.
Coordinate and monitor the delivery of supports, ensuring services are implemented effectively as outlined in the plan.
Advocate for participants’ rights and choices, empowering them to voice their needs and make informed decisions.
Review progress regularly and adjust support strategies as needed to help participants move toward greater independence and well-being.
Maintain organized documentation and reports to track participant outcomes and meet NDIS requirements (so every success is recorded and celebrated!).
About You – Skills, Experience & Passion
You are a proactive, compassionate professional who truly cares about making an impact. You thrive on helping others and have the skills to keep things organized and on track. To be successful in this role, you will bring:
Experience in the disability sector, case management, or community services (NDIS support coordination experience highly regarded).
Solid understanding of the NDIS and how to help participants get the most out of their NDIS plans.
Excellent communication and interpersonal skills – you build trust with participants, families, and service providers alike.
Strong organizational and time-management skills to juggle a caseload and keep on top of documentation and deadlines.
A knack for problem-solving and the ability to work independently – you take initiative and can make informed decisions to support your participants.
Empathy, integrity, and a participant-first mindset that align with DSP’s core values.
Necessary qualifications/checks: NDIS Worker Screening Check, Working With Children Check, and a valid driver’s license (or willingness to obtain these).
A relevant qualification in Disability, Social Work, Community Services or similar is a bonus – but passion and experience matter most!
