Key Responsibilities:
Event and Activity Coordination:
Organise and coordinate events and activities for participants, ensuring a diverse and engaging program.
Identify a variety of group options (internal and external within an agreed budget) and incorporate them into a monthly site program.
Create engagement forums for participants and staff to foster a sense of community and involvement.
Evaluate participants' goals and create tailored activity plans, contributing to individual care plans and reporting processes.
Complete service agreements with participants, ensuring all necessary documentation is in place.
Manage and update a register of all activity equipment, ensuring resources are well-maintained and accessible.
Social Media and Communication:
Take charge of social media platforms, posting updates and engaging with the community.
Create newsletters for aged care and disability services to keep participants and staff informed.
Complete feedback loops with participants and staff, compiling reports and analysing trends for continuous improvement.
Assist with organising meetings and increasing attendance through effective communication strategies.
Take turns with meeting minutes and assist in organising meetings to increase attendance.
Administration and Compliance:
Complete service agreements and SIL Rosters of Care (ROC).
Manage compliance registers, reporting quarterly to the team.
Ensure activities are culturally appropriate and responsive to individual needs, fostering inclusivity.
Maintain contacts with families and stakeholders, building and managing partnerships in the community.
Ensure the team and activities function with a clear awareness of the organisation’s OH&S policies and address any risks or potential hazards immediately.
Report any updates and changes to the director on a regular basis.
Manage rostering coordinators, overseeing and supervising their activities, and reporting to the director.
Ensure compliance and adherence to policies for the program.
Complete quarterly Aged Care newsletter.
Financial and Reporting Responsibilities:
Budget and complete quarterly reports on income generated from the program.
Actively recruit participants to sustain program viability and oversee rostering coordinators.
Manage and complete the quarterly report on program income, ensuring financial sustainability.
Submit fortnightly reports to Judith on payroll week for Support Coordination billables.
Training and Development:
Organise training sessions on Flowlogic and rostering systems.
Be willing to be trained in case management, end-of-year reports, and care plan implementation.
Contribute to the continuous improvement register, identifying and implementing enhancements to the program.
Participant and Staff Engagement:
Create and manage engagement forums for participants and staff to discuss needs, feedback, and improvements.
Ensure services are culturally appropriate and responsive to individual needs, fostering an inclusive environment.
Organise staff acknowledgement initiatives, recognising and celebrating staff contributions.
On-Call Duties:
Participate in an on-call rotation to provide support and address any urgent issues as they arise.
Qualifications:
Diploma or higher qualification in a related field.
Current Blue Card, NDIS Screening, First Aid, CPR, Police Check, Driver’s Licence required.
Relevant industry certifications (e.g., case management or aged care-specific training) will be highly regarded.
Proven experience in event planning and coordination, preferably in aged care or disability services.
Strong organisational and communication skills, with the ability to engage diverse groups.
Strong and demonstrated understanding of rostering, preferably in aged care or disability services.
Understanding of compliance requirements and policies within aged care or disability services.
Proficiency in social media management and community engagement strategies.
Ability to work collaboratively within a team and independently manage responsibilities.
Demonstrated ability to manage budgets and financial reporting.
Experience in training and staff development is preferred.
Experience with Flowlogic/Datanova systems is advantageous but not necessary.
Must have experience with Microsoft Office programs e.g. Excel, Word, etc. Particularly, strong proven knowledge working with Excel.
Working Conditions:
Full-time position with rotating on-call duties.
Flexible schedule to accommodate participant and staff needs.
Office-based work with field visits and community engagements.
