Brisbane North / Sunshine Coast Position – this position is based in Brisbane however will require oversight and travel to the Sunshine Coast region
Our Organisation
We are small energetic business with a background in Plan Management, who has recently established a Support Coordination Team. We are seeking another self- motivated, enthusiastic member to join alongside the current team to continue to provide a personalised approach to the Support Coordination services currently offered.
The Role
We are seeking a positive, well-organised individual who is self-motivated and a strong communicator. You will need to be enthusiastic, easy going and with a sound understanding of the NDIS.
As Support Coordinator you will focus on collaborating and assisting people with disabilities predominately in the Brisbane region. Your responsibility will be to ensure the implementation of participant's plans and the achievement of their goals, building better outcomes for the lives of the Participants you support.
Your skills and knowledge
Experience in Support Coordination, case management or a similar role.
Knowledge of practice and guidelines as they relate to the NDIS.
A sound understanding of and experience in providing/coordinating inclusive community-based services for people with disabilities
Experience in assisting management of complex enquiries with competing priorities and deadlines
Tertiary qualifications in a relevant humanities field are desirable
Initiative, strong attention to detail and problem-solving skills
Ability to analyse problems and provide meaningful solutions
A Current Driver’s License and use of a roadworthy vehicle
Key facts about the position:
· Full time, Salary is based on the Social, Community, Home Care and Disability Services Industry Award 2010 Level 4 (Pay Point Range 1–4). Rate of pay will be based on qualifications and experience.
· This position requires travel between Brisbane North as far as Sunshine coast
