The Role
Maternity leave - 12 month fixed-term contract - 01 May 2025 - 30 April 2026 / with possible extension - Monday to Friday with Flexible start and finish times - 76 hours per fortnight. Travel required across Sydney (mainly Western Sydney) - Mileage claim or pool vehicle available.
To attend to all clinical care needs of clients in accordance with their care plan. This will be achieved by providing safe and effective direct nursing care in line with nursing best practice principles. This position is also responsible for assisting with and overseeing the education of Home Care Employees in clinical matters as required.
Your role will include:
Carry out the initial and on-going assessment, planning and management of care for clients
Review all clients frequently and document clients file
Provide, or ensure provision of, more complex care services including:
Pain management
Palliative care
Naso-gastric tubing
Catheter care
Stoma care
Complex wound management
Participate in their annual care plan evaluation, or more regularly as required
Ensure that all documentation required by and related to the provision of quality care for all Clients is prepared, maintained, monitored, evaluated and up to date in accordance with SCC policies and procedures.
Act as a consultant in response to the complex nursing and care needs of Clients
Lead and/or be involved in staff development, education and training sessions for Home Care Employees
Deliver relevant clinical education and coaching and mentoring to indirect reports
Liaise with and advocate on behalf of Clients to medical and allied health professionals
Provide support and guidance to Home Care Employees, Clients, their families and supporters
As require, lead and/or be involved in case management discussions
Develop personal care plans that ensure the individual needs of Clients are identified and met, and be aware of changes in care plans
Ensure clients incidents of a clinical nature are reviewed and actioned
Keep the Home Care Coordinator informed of any changes in each Client’s appearance, emotional or physical condition, as well as any behavioural changes or problems.
Communicate appropriately with Clients while working with them.
Respect the rights of each Client to maintain their religious and cultural links as appropriate
Carry out other reasonable duties which are consistent with the role of this position, as requested by supervisory staff
Essential
Registered Nurse (Division 1) with general registration
Desirable
Possession of Certificate III in Community Services (Aged Care) or equivalent
Experience & Skills
Essential
Experience in caring for older people with the ability to provide quality care and support.
Current driver’s license
Sensitivity to and understanding of the aspirations and needs of frail older people.
Ability to set and maintain high standards in the provision of quality care and support services.
Excellent interpersonal and written communication skills.
Advanced verbal and written English communication skills.
Numeracy skills - reconciliations and statistics.
Sound understanding of relevant State and Commonwealth legislation as it applies to Aged Care providers
