We are currently looking for a Groups Service Coordinator to join a small team focused on quality outcomes. You will assist and support NDIS participants, their families and carers to enable them to implement their goals and aspirations.

To write, guide and deliver a program assisting our participants to achieve their goals based in the Holistic Horizons Hub and Activities Rooms. The incumbent also provides independent direct care assistance and support for clients as Support Worker in 1:2 ratios or higher. This position is PPT/Cas with 15 – 20 hours week min up to 30 hours at SCHADS Level 2.4.

Working as part of a disability support team the Program Leader will maintain an effective and consistent approach to service delivery under the general direction of the NDIS Service Manager. The position requires the person to have a commitment to the aims and objectives of Holistic Horizons Support Services.

Groups Service Coordinator Responsibilities and Duties.

· Write a program that provides documented progression and activities based on participant NDIS goals.

· Monitor, evaluate and review program outcomes.

· Provide site leadership and ensure staff compliance with established organisational policies and procedures.

· Undertake a range of activities requiring exercising of initiative in application of established work procedures.

· Liaise with guardians / stake holders / participants to organise social outings.

· Develop responses to client issues using initiative and judgment under broad direction of the Team leaders and other stakeholders.

· After hours On-call phone on a rotating roster.

About you:

· You are an experienced support worker. You currently hold a Cert 3 in Individual Support, Disability or a related discipline or be working towards higher qualifications in Disability or Community Services.

· Previous experience writing and implementing programs for disability services.

· You have demonstratable skill sets in other areas e.g. Fitness, Music, Art, Cooking.

· You are adaptable, think on your feet and can problem solve effectively.

· You are aware of risk and danger and can keep yourself and your client safe, while assisting them to try new things and take risks to assist them to grow and learn.

· You can advise your staff in safety measures and supporting safe practices including restrictive practices.

· You’re a proactive support worker, who meets all the requirements of that role.
