We are seeking a Case Coordinator to assist in stakeholder engagement for our SIL Accommodation Services. Our SIL Accommodation Services supports people living with complex and enduring disabilities, with many participants requiring 24-hour support.

About You:

We're looking for someone who:

  • Demonstrated ability to manage stakeholder relationships
  • Advanced written and verbal communication skills with an ability to develop a professional rapport with a diverse range of people
  • Excellent communication skills
  • Ability to empathise with participants and families
  • Ability to handle crises
  • Excellent problem solving and conflict management skills
  • Incident response and reporting procedures and protocols to ensure the participant's safety, self, and others
  • Reflective practices and processes
  • Working within current best practice tools, approaches and expectations about disability and related support.
  • Strategies and tools to manage work stress and manage personal wellness
  • Experience writing assessments and reports to monitor participant progress
  • Experience working with individuals with a range of disabilities including Autism Spectrum Disorder (ASD) Level 3, Intellectual Impairment, and other complex diagnoses
  • Understanding and practical application of relevant legislative regulations and NDIS Standards and Rules
  • Demonstrated ability to manage a caseload efficiently and effectively

Key Responsibilities:

  • Coordinate the onboarding of new Participants to ensure a smooth transition, including the completion of all administrative/compliance requirements
  • Work closely with participants, their families, and other internal/external stakeholders to deliver high-quality and person-centred care on a day-to-day basis
  • Attend and participate in regular stakeholder meetings
  • Review of Participant records such as Incident Reports, Behaviour Data, Sleep Data, and other recordkeeping relevant to the Participants' needs.
  • Assist in the development of Support Plans
  • Work closely with SIL Team Leaders and Operations Managers to deliver positive outcomes for participants
  • Report to Senior Management
  • Maintain accurate records in compliance with policies, regulations and legislation
  • Complete reports and work towards goals within the NDIS plans
  • Working within NDIS and government legislative requirements

Essential Criteria:

  • Excellent verbal and written communication skills
  • Knowledge of relevant legislation, policies and procedures impacting the delivery of services to people with a disability
  • Current NDIS Worker Screening
  • New Worker – NDIS Induction Module
  • Willing to transport participants where required
  • Hold a current First Aid/CPR certificate or be willing to obtain prior to commencement
  • Enthusiastic about supporting people with psychosocial disabilities, intellectual disability, and/or mental health needs

Preferred Criteria:

  • Experience in NDIS Support Coordination / Case Management
  • Qualification in Social Work, Human Services, Occupational Therapy or other Allied Health degree
  • Understanding of Positive Behaviour Support
