About the role

We are expanding our team! Join Pivot Support Services' dynamic and caring NDIS team as a Support Coordinator in our Albany, WA office. As a Support Coordinator, you will play a key role in enabling NDIS participants to live independently and thrive in their communities. This full-time position offers opportunities for professional growth and development within a supportive, values-driven organisation.

What you'll be doing

Develop and coordinate individualised support plans to help clients achieve their goals

Liaise with service providers, community groups and other stakeholders to arrange and monitor client services

Advocate for client needs and ensure they receive high-quality, person-centred care

Regularly review and update client support plans to reflect changing needs and circumstances

Maintain detailed client records and documentation in accordance with organisational and legislative requirements

Provide guidance and assistance to clients, their families and carers

Identify opportunities to improve service delivery and recommend enhancements

What we're looking for

Relevant tertiary qualification in community services, disability support or a related field

Preferred - experience in a similar support coordination or case management role

Strong understanding of the disability support sector

Excellent communication, interpersonal and advocacy skills

Ability to work collaboratively with a diverse range of stakeholders

Strong organisational and time management skills

Empathetic, person-centred approach and commitment to delivering high-quality client outcomes

Current driver's licence

NDIS Worker Screening Check or willingness to undergo screening
