Clinical Care Coordinator - Central Coast - Aged Care

We are currently recruiting for a Clinical Care Coordinator in the Beautiful Central Coast of NSW for a Leading Aged Care Provider!

The Role:
As a Clinical Care Coordinator, you will manage and oversee the clinical care of all residents, ensuring documentation and assessments are consistently up to date and aligned with best clinical practices.

You will supervise afternoon and weekend shifts, ensuring the highest standard of care is delivered while providing strong leadership to the team.

Requirements:

Clinical Care Coordinator experience or RN in Charge ready to step up

Minimum of 5 years’ experience as a Registered Nurse

Strong leadership experience and ability to mentor and support Registered Nurses

Valid NDIS Workers Screening Clearance (or ability to obtain)

Strong knowledge of Aged Care assessments, care plans, AN-ACC, and accreditation compliance
