We are seeking a highly experienced Specialist Support Coordinator to make the step into a Clinical Lead role in our growing Support Coordination team in Perth. The role is full time, but negotiable for the right person. It is a Hybrid role, and flexible working arrangements are promoted!

About the role

Reporting to the Directors of Future Focus Support Coordination, the primary objective role of the Clinical Lead is to strengthen the practice response, support and risk management accross our organisation. You will be required to deliver Clinical Supervision, particiapte in risk reviews for your assigned region, provide clinical debrief and support the ongoing professional development on the Support Coordinators in your care. You will also support a small caseload of 5-10 particiapnts, with complex needs. You will undertake some face to face meetings, case conferences, manage NDIS budgets, refer accross service systems, and help your participants successfully implement their NDIS plan.

In their direct work, the Clinical Lead will focus on strengthening a their team's ability to navigate intersecting service systems, including Health, Justice, Mental Health, Housing, and other Social Services. The incumbent will contribute to the development of evidence-based and innovative clinical services and that will promote the delivery of Support Coordination of the highest quality.

Please only apply if you hold:

• Bachelors level qualification in Social Work, Social Science, Health Science, Case Management, Mental Health/Psychology or other Allied Health qualification.
• Previous experience supporting participants in the NDIS that have mental health, psychosocial or disability support needs in a case management style role.
• Extensive knowledge of disability, mainstream and community services.

To be succesful in this role you must:

• Currently living in Perth or surrounding suburbs
• Have a valid Driver's Licence
• Have access to reliable vehicle
• Support a strengths-based approach to working with disability
• Able to successfully pass probity checks: International Police Check if relevant, Working with Children Check, NDIS Worker Screening Check

Advantageous Skills and experience:

• A self-starter, self-motivated person who can work independently
• Previous Specialist Support Coordination Experience
• Demonstrated knowledge of psychosocial diagnoses, both chronic and acute, or the ability to source accurate and critical patient information.
• Solid background and knowledge of NDIS
• Excellent verbal communication skills and a demonstrated ability to engage with participants and their families
• Strong written and computer skills for the recording of notes, gathering evidence, writing reports, correspondence and other documents
