About the Role

Are you a passionate and dedicated support coordinator looking for an opportunity to make a meaningful impact in the lives of individuals within the National Disability Insurance Scheme (NDIS) and Aged Care?

We invite you to join our dynamic team as an independent and autonomous Support Coordinator or PRC who is solely focused on delivering Support Coordination or PRC services rather than billable hours!

This role is specifically created for individuals with 4+ years professional experience and service delivery in their role and are already looking for a role & a Provider with a Difference.

Why now?

Anticipating and watching the regulatory changes in the Support Coordination space over the last 3 years in particular, concerned to see many providers leave the industry and even more independent’s needing to pursue registration we have chosen to act and became a NDIS Registered Provider for Specialist Support Coordination & Support Coordination in January 2025.

Our goal is not to deliver SSC, SC & PRC services ourselves but more so to facilitate the army of skilled and passionate Coordinators and Coaches that find themselves either burdened and focused on KPIs to build their own independent service, benefiting themselves, their participants and the community on the whole

Due to the our continued and unprecedented growth; moved into our brand new premises with 5 clinic rooms, community / group space and home our administration in May 2024 we can now confirm our South West expansion with our Busselton office opening in January 2025!

It is our belief that this enables the therapist to be placed not only at the heart of assisting the participant in their journey to deliver on their goals, but also for the therapist to act and build their own therapy business without all the hassle of running their own business.

Responsibilities

• Provide high-quality Coordination of Support or Psychosocial Recovery Coaching to clients in the NDIS.
• Conduct comprehensive onboarding and intake processes to ensure an effective and compliant support planning journey.
• Collaborate with a multidisciplinary team to support client goals and outcomes.
• Monitor and evaluate client progress, adjusting supports as necessary.
• Maintain accurate and up-to-date client records in compliance with regulatory standards.
• Engage in continuous professional development to stay current with best practices in Coordination of Supports.

Requirements

• Preferably tertiary educated or in an appropriate discipline/qualification as required by NDIS auditor certification standards.
• At least 4 years relevant professional experience.
• Experience working within the NDIS.
• Excellent communication, interpersonal, time management and organisational skills.
• Ability to work independently as well as part of a team.
• Current driver's license and willingness to travel as required.
