As we continue to grow, we’re looking for dedicated Support Coordinator to join our team and contribute to meaningful change in the lives of people living with disability.
About the Role
We are currently seeking an experienced and compassionate NDIS Support Coordinator to join our team. In this role, you’ll support participants to understand and implement their NDIS plans, connect with services, and work towards achieving their individual goals.
You’ll be instrumental in helping clients access the right supports, advocate for their needs, and navigate the NDIS system with confidence.
Key Responsibilities
- Develop and implement individualized support plans based on NDIS goals.
- Coordinate access to services including allied health, accommodation, employment, and community participation.
- Provide ongoing support and guidance to participants and their support network to ensure goals are achieved and services are delivered in line with their NDIS plan.
- Build strong relationships with participants and key stakeholders.
- Advocate for participants during plan reviews and service negotiations.
- Monitor participant progress and adjust plans as needed.
- Maintain accurate records, progress reports, and consistent communication with all parties.
- Work collaboratively with internal teams and external providers to deliver integrated support.
- Contribute to the continuous improvement of support coordination practices at DCA.
What We’re Looking For
- Minimum 2 years’ experience in support coordination or a similar role in the disability or community services sector.
- Deep understanding of the NDIS framework and plan implementation process.
- A solid understanding of quoting, budgeting, and financial procedures, with strong financial acumen.
- Excellent communication, organisation, and problem-solving skills.
- Ability to work independently and manage a diverse caseload.
- Relevant qualifications in social work, disability, or related fields.
- Current driver’s licence and access to a reliable vehicle.
- High level of empathy, professionalism, and cultural competency.
