Role Overview:
We are looking for an enthusiastic, compassionate, dedicated and experienced Support Coordinator to join our team. In this role, you will play a key part in assisting NDIS participants to implement their plans, connect with their communities, and manage their supports effectively. Your focus will be to build the participant's capacity to navigate the NDIS system, resolve challenges, and achieve their goals.
Key Responsibilities:
- Assist NDIS participants in implementing their plans and increasing their capacity to self-direct their supports.
- Guide participants in exploring mainstream, community, and informal support options.
- Negotiate services and pricing with service providers, and develop service agreements.
- Collaborate with plan managers to ensure funding is allocated appropriately.
- Provide participants with support in resolving issues with service providers, understanding their responsibilities, and preparing for plan reviews.
- Assist with transitioning and housing planning, as well as providing education related to NDIS plans.
- Foster strong relationships with participants, families, carers, and service providers.
Key Requirements:
- Strong interpersonal and communication skills with the ability to adapt to individuals' needs.
- High level of numeracy and literacy skills, with excellent attention to detail.
- Ability to resolve conflicts and manage complex situations.
- Experience working with NDIS participants and understanding NDIS funding and legislation.
- Excellent administrative and organisational skills, with strong time management capabilities.
- Competency in using computer systems and technology to manage participant information.
- A commitment to confidentiality and professional ethics.
- Ability to work autonomously and as part of a supportive team.
