About Us?
The Institute for Urban Indigenous Health Ltd (IUIH) was established by its founding members to provide a coordinated and integrated approach to the planning, development, and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Queensland region.
The Staying Deadly Hubs provide comprehensive assessment, treatment, and assertive outreach support to clients presenting with moderate, and/or severe mental health needs. Operating within a unique multidisciplinary care team, the Hubs extend IUIH’s comprehensive model of care by integrating with Staying Deadly Hub teams to deliver appropriate and responsive care to clients who may otherwise not access support or require inpatient care.
About the Role?
The Staying Deadly Care Coordinator provides outcome-focused health, social, and practical support to Aboriginal and Torres Strait Islander clients, with a strong emphasis on psychosocial well-being. This role works collaboratively within a multidisciplinary team alongside clinicians to address the mental health needs of clients experiencing moderate to severe conditions.
Clients supported in this role may be at risk of entering the acute care system if their symptoms remain unmanaged or may be transitioning from inpatient care, requiring continued support in the community. The Care Coordinator ensures that clients receive holistic, wrap-around primary health and mental health care to promote recovery, resilience, and well-being.
This position involves supporting people with diverse needs, empowering them through strength-based and culturally responsive care planning, coordination, and advocacy. Working in close collaboration with clinicians and other stakeholders, the Care Coordinator fosters integrated, person-centred support to enhance client outcomes.
Key Responsibilities:
- Work collaboratively as part of a multidisciplinary team to ensure integration and coordination across different programs.
- Develop, implement, and maintain support plans in partnership with clients and relevant service providers.
- Utilise trauma-informed, recovery-oriented, and strength-based approaches to formulate, implement, and review well-being plans, including comprehensive assessments and safety planning.
- Assist in the coordination and delivery of services for clients experiencing mental health concerns, substance use issues, chronic disease, and social health challenges.
- Facilitate care coordination activities to support clients’ transition care plans, including participation in meetings with relevant internal and external stakeholders.
- Provide flexible and responsive support tailored to client and family/carer needs.
- Conduct home, community visits to support ongoing client recovery and engagement in care.

- Current Blue Card
- Current Criminal History Check
- Current CPR and First Aid Certificate (Willing to obtain)
- Current C Class Drivers Licence (Qld)
- Certificate IV or Higher in relevant field
Desirable, but not mandatory:
- Demonstrated experience working with Aboriginal and Torres Strait Islander people and /or communities
