About the Role?
Working with Mob Link provides the opportunity to work across the growing IUIH network, to provide social health care and coordination to mob across the lifespan from jarjum to elders, collaborating with the Inner City Referral Service, Staying Deadly Hubs, Child Youth and Family Health Team, Primary Health Care Clinics as well as close collaboration with external stake holders across SEQ Hospital and Home Services.
The purpose of the position is to be a comprehensive linkage and coordination service to address gaps in care for Aboriginal and Torres Strait Islander people at the interfaces between home/community, primary care, and secondary and tertiary care services. Connecting clients to secure and stable housing assistance, mental health support, Domestic and Family Violence and other Social Health supports as needed. Coordinate clients with complex needs between IUIH community and hospital services for all ages; paediatrics through to our elders.
The position aim is to reduce the significant points of risk in a person’s journey through the health care system, in particular, at hospital entry and discharge, and in transit from primary care in and out of specialist, allied health, and other services.
Day to Day duties may include:
- Work with relevant health care providers, specialists, and GPs to manage a short-term rolling caseload of more complex clients requiring social and mental health care coordination support
- Providing timely handover and connection to relevant health care and primary care providers, ensuring effective communication throughout
- Facilitate coordinate case conferences to ensure wrap around care and supports are provided
- Attend clinical case reviews with Senior Care Coordinators
- Contribute support and advise to provide more culturally appropriate care to patients attending external practices/health care provider
- Support designated health service providers and staff to implement referral systems, procedures, that improve the quality of service to Aboriginal and Torres Strait Islander clients especially those with or at risk of chronic disease.
- Coordinating as well as supporting specialist and allied health visits by referred clients.
- Keep accurate, current case notes, statistics and other client records and files, as required.
About You?
- Current C Class Drivers Licence (QLD)
- Current Criminal History Check
- Minimum Diploma of Health Services / Social Service / Social Sciences or relevant qualification
- Valid Blue Card
- Valid Yellow card
Desirable, but not mandatory:
- Demonstrated experience working with Aboriginal and Torres Strait Islander people and /or communities
