Your Opportunity
Located in South Hedland, the Drug and Alcohol Treatment Service (DATS) is a community-based program dedicated to supporting young people aged 12-25 in the Pilbara. This service provides targeted interventions to help young people develop resilience, build confidence, and navigate challenges linked to substance use. Operating under a trauma-informed, harm-reduction model, DATS delivers specialist youth counselling, family support programs, outreach services, and community-strengthening initiatives.

As a Case Manager, you'll work directly with young people facing complex challenges, offering structured case management and advocacy. You'll play a crucial role in assessing needs, developing individualised support plans, and working alongside families, communities, and service providers to ensure the best outcomes. The role demands initiative, sound judgment, and the ability to manage multiple priorities in a fast-paced environment.

Your Key Responsibilities

  • Comprehensive Case Management - Deliver person-centred case management to clients, focusing on their immediate and long-term needs while aligning with Mission Australia's National Case Management Approach.
  • Intake, Assessment & Intervention - Conduct detailed assessments, establish case plans, and implement targeted interventions to support recovery and reduce risk-taking behaviours.
  • Outreach & Community Engagement - Provide mobile support across the Pilbara, building trust with young people, their families, and key stakeholders.
  • Advocacy & Stakeholder Collaboration - Establish strong referral pathways, attend interagency meetings, and advocate for clients to ensure access to essential services.
  • Capacity Building & Life Skills Development - Work with clients to foster independence, self-management, and long-term well-being through tailored support plans.
  • Administrative & Reporting Duties - Maintain accurate case notes, complete reports, and ensure compliance with all legal and organisational requirements.
  • Team Collaboration & Continuous Improvement - Contribute to team development, mentor colleagues, and support ongoing improvements in service delivery.

Requirements for Success
With a significant background in AOD, Community Services and/or Human Services sectors, you will be confident in your case management and assessment processes. Other requirements include:

  • A relevant qualification in psychology, social work, human services, or a related field, or extensive relevant experience.
  • Demonstrated ability to apply trauma-informed care and harm reduction strategies in case management.
  • Proven experience in the alcohol and other drugs (AOD) or family and domestic violence (FDV) sectors, ideally with at least one year in a frontline role.
  • Strong interpersonal skills with the ability to engage young people in challenging circumstances using a person-centred approach.
  • Ability to work remotely, manage a complex caseload, and respond calmly under pressure.
  • Well-developed administrative and organisational skills, with the ability to manage time effectively and meet reporting requirements.
  • A current Driver's Licence, with a willingness to travel for outreach work.
