Your Opportunity
Our Integrated Homelessness Supported Accommodation Program (IHSAP) also known around the region as "Batten Road", is a collaboration of complementary services and agencies that have established co-located specialist supports such as Mental Health, Alcohol and Other Drug, Specialist Family Support and other specialist homelessness providers working together to support our communities homeless population. Mission Australia provides client engagement and case coordination services for people who are living at the facility.

In this vital role, you will support clients and colleagues by delivering services aligned with Mission Australia's values, contributing to impactful outcomes within IHSAP. As a Case Manager, you will provide direct support through case management, informal counselling, and life skills training, empowering individuals to face homelessness and tenancy risks while pursuing secure long-term accommodation.

Your Key Responsibilities

  • Conduct comprehensive client intake and risk assessments, developing tailored case management and support plans aligned with immediate and long-term needs.
  • Provide person-centred case management using Mission Australia's National Case Management Approach, ensuring cultural sensitivity and responsiveness.
  • Facilitate referrals and connections to specialist services, including healthcare, AOD, mental health, and housing providers, to support holistic client outcomes.
  • Assist clients in transitioning into independent living or unsupported tenancies, fostering self-management and sustainable housing practices.
  • Build and maintain professional relationships with community organisations, government and non-government agencies, and Indigenous organisations.
  • Complete administrative tasks, including case notes, reports, data entry, and maintaining accurate client records within the client management system.
  • Participate in inter-agency meetings, service promotion events, and quality improvement initiatives to enhance program delivery and client outcomes.

Requirements for Success
With a significant background in Homelessness, Community Services and/or Mental Health sectors, you will be confident in your case management and assessment processes. Other requirements include:

  • Relevant qualifications (e.g., degree or diploma) with professional experience in case management and homelessness services, including a minimum of 2 years working with vulnerable clients.
  • Proven ability to manage a caseload of clients with complex needs, demonstrating trauma-informed practice and a strengths-based approach.
  • Experience and knowledge of Aboriginal cultural systems and protocols, with a capacity to build respectful and professional relationships.
  • Strong written and verbal communication skills, including the ability to manage administrative tasks and maintain professional boundaries.
  • Resilience and adaptability when working with clients experiencing distress, trauma, AOD use, and aggressive or uninhibited behaviours.
  • Proficiency in Microsoft Office applications, including Outlook, Word, Excel, and PowerPoint.
  • A commitment to person-centred approaches and a belief in individuals' capacity for recovery and growth, informed by lived experience and professional development.
  • Full driver licence with no restrictions
