Ability Choice Care is a rapidly growing NDIS provider, located in a prime location in Westpoint Blacktown. We are an innovative and energetic disability organisation committed to improving the lives of people with disability throughout the Western Sydney. We offer a range of services including a “State of the Art” Activity Centre, innovative community supports and modern Supported Independent Living houses, in quality environments.
We are seeking a Full-Time experienced Support Coordinator to join our growing team. On successful completion of a qualifying period, this will be a permanent position.
The NDIS Support Coordinator is responsible for planning and organising support for people with disabilities within our centre, their home, shared accommodation and in the community to enable them to achieve their goals and live fulfilling lives. NDIS Support Coordinators lead and consult daily with a team of support workers responsible for maintaining an environment that provides safety, quality of life, and empowers people with a disability through choice and control principles.
What you will be doing:
Your primary role will be to collaborate directly with our participants, guide, listen and support them through coordinating and managing their NDIS plan, budgeting, and consulting with providers and stakeholders.
Managing a caseload of participants rated with varying levels of complexity and plan literacy.
Creating resources related to the individual needs of the participant.
Training staff, modelling, and building skills, competencies, and knowledge within the support provision team.
Setting in place strategies and interventions to support clients who present significant risks to themselves or others: e.g., using best practice training methods, and external resources such as behavioural support practitioners, therapists, and paramedical staff: e.g. Psychiatrists and Clinical specialists.
Coordinate extensive evidence gathering with the aim of supporting clients to obtain appropriate levels of funding. Proofing and checking all submissions before they are uploaded to the NDIS portal.
Participate in the benchmarking and continuous improvement of Support Coordination documentation, actively contributing ideas and discussing improvements during monthly Support Coordination team meetings. Consulting with internal subject matter experts to improve standards of recording, competence, and reporting by Support Worker staff.
To be considered, you will have:
Graduate or post graduate level of qualifications in a relevant field and a minimum of 2 years’ experience working complex clients. E.g. Social Science or Health Science qualifications such as Social Welfare, Psychology, Social Work, Nursing or Disability Education.
OR
Diploma level qualification in a relevant field with a minimum of 2 years’ experience collaborating or case managing a range of complex clients.
OTHER REQUIREMENTS:
Demonstrated ability to work autonomously, manage multiple tasks and outcomes, within prescribed timelines and deadlines.
Experience collaborating with clients from culturally and linguistically diverse (CALD) backgrounds, displaying cultural competence and understanding the need for agility in service delivery.
Some experience collaborating with clients involved with the criminal justice system, state welfare system or public housing circumstances, recognising the unique challenges and support needs of people living with a disability in this context.
Ability to confidently hold fellow service providers accountable if support is not aligned with client goals and preferences, and the ability to resolve or escalate the situation (internally or externally) according to participant needs.
Demonstrated knowledge of disability types, resources and standard assessments that can be used to measure intellectual, physical, or psychological barriers and their impact: E.g., CANS, WAIS R, WHODAS, MIMS, DSMV5 etc.
Demonstrated ability to collaborate with parents, carers, or guardians who are experiencing burnout or struggling with decision making, providing intensive support to ensure the best possible outcomes for clients.
Demonstrating a keen sense of resilience, ethical practice, and personal accountability.
Ability to introduce structure and promote collaboration within the Support Worker community. I.e., be responsible for ensuring quality supports whilst maintaining individual accountability from the workforce.
Ability to adapt, create and implement person-specific resources and documentation to meet the emerging needs of clients, ensuring individualised and effective support.
Financial literacy and a strong connection with the rostering and planning team throughout the business.
OTHER:
Current NSW Driver’s License
Current First Aid Certification.
High Level of understanding of the National Disability Insurance Scheme and its purpose
NDIS Workers Check Clearance
Working with Children's Check Clearance
National Police Check
Excellent organisational and time management skills
