About the Role

We are seeking a compassionate and motivated Support Coordinator to join our team. As a Support Coordinator, you are responsible for proactively assisting Deaf, Deafblind, and hard-of-hearing customers in utilising and managing their NDIS plans.

Furthermore, this role will also mentor and support team members to ensure excellence in service delivery, advocate for clients’ needs, and promote capacity building and independence among participants.

Main responsibilities of the role include:

  • Manage a caseload of complex cases, providing high-level support coordination to participants.
  • Promote community awareness of available services and support options.
  • Track, monitor, and report hours against each client’s allocated budgets.
  • Mentor and guide Support Coordinators to deliver person-centered, high-quality services in line with participant goals.
  • Facilitate referrals to internal and external services as required, ensuring participants’ needs are prioritized.
  • Support the Manager of Support Coordination in ensuring the team’s goals are met, escalating accordingly when KPI’s are at risk of not being met.
  • Lead team knowledge-sharing sessions and contribute to continuous improvement initiatives.
  • Identify opportunities for participants to connect with community services and programs that enhance their social and economic participation.
  • Act as a point of escalation for team members and participants, ensuring swift resolution of issues.

About You

In your application, let us know if you have these key skills and experience:

  • Experience in a senior or similar supervisory role within the disability sector.
  • Demonstrated experience in support coordination, case management, or a similar role.
  • Fluency in Auslan
  • Extensive knowledge of, and awareness of the Deaf community and culture.
  • High level of numeracy and literacy skills.
  • Advanced Microsoft office skills, especially using excel.
  • Ability to work both independently and as a part of a team
  • Passionate about provision of high-quality person centred supports for our Deaf and hard of hearing individuals
  • Ability to build and maintain strong community connections and relationships with service providers, individuals and/or families/ carers.
  • Completed NDIS Worker Orientation and NDIS Worker Screening Check (or commitment to obtain).

You should also let us know if you have these other great skills:

  • Relevant qualification such as a minimum of Cert IV in Disability Services
