About the role We are seeking a Youth Care Coordinator to support the headspace Goulburn team in delivering high-quality mental health services to young people.
This is a full-time role, currently available until 30 June 2026, with the potential for extension based on continued funding. The position will be based in Goulburn.
This position offers the opportunity to:
- Work collaboratively with young people and their friends, family and carers to identify their support needs, goals and develop collaborative care plans.
- Provide short term care coordination, deliver early intervention counselling and support services
- Work closely with the other members of the headspace Goulburn team and external service providers to coordinate support needs.
We are looking for someone with:
- Qualifications in a relevant field or working towards same e.g. youth work, community welfare, social work, psychology, peer work, occupational therapy, counselling or other related qualifications or demonstrated experience in a similar role.
- Demonstrated understanding of issues affecting the health and wellbeing needs of young people with mental health and/or substance use problems, as well as young Aboriginal people and their families.
- Experience delivering individual and group interventions
- Experience providing coordination of care and ability to work from a recovery-oriented framework.

About us Grand Pacific Health is the lead agency of headspace, Goulburn. We are a not-for-profit, primary health care organisation providing locally-tailored, high quality physical and mental health services across NSW and the ACT. Headspace Goulburn provides early intervention mental health services to young people aged 12-25 who have or may be at risk of developing a mental health and/or substance use disorder or have economic and social recovery needs. It is a part of the Youth Mental Health Team within Grand Pacific Health (GPH).
