NDIS Service Coordinator

Location: Western Suburbs (with opportunities for remote/hybrid work)
Employment Type: Part-Time
Reports to: Operations Manager

We provide tailored support services, including Supported Independent Living (SIL), Social and Community Participation, Assistance with Daily Life, and Medium-Term Accommodation (MTA). Based in the western region of Victoria, our services extend across the state, making a meaningful impact on the lives of our participants.

We are seeking a compassionate and organised Service Coordinator to join our dedicated team. This role is crucial in ensuring the smooth coordination of support services for our NDIS participants.

Role Overview:

The Service Coordinator is responsible for managing the delivery of high-quality services to participants. This role involves coordinating with support workers, participants, families, and other stakeholders to ensure participants' needs are met effectively and in line with their NDIS plans. The successful candidate will have excellent organisational and communication skills, a deep understanding of the NDIS, and a passion for making a positive impact in the community.

Key Responsibilities:

  • Coordinate the day-to-day delivery of services, ensuring all participants receive appropriate and timely support.
  • Develop and maintain individualised service plans for participants in line with their NDIS goals and funding.
  • Act as the main point of contact for participants, families, and support coordinators, addressing any concerns or queries.
  • Liaise with support workers to ensure they are equipped with the necessary resources and information to deliver services.
  • Monitor service delivery to ensure compliance with NDIS standards, regulatory requirements, and organisational policies.
  • Maintain accurate records, update participant progress, and ensure that documentation is completed on time.
  • Collaborate with the broader team to continuously improve service quality and participant outcomes.
  • Coordinate participant intake, assessments, and service reviews, ensuring all services align with their goals.
  • Work closely with external agencies and service providers to coordinate additional support where required.

Qualifications & Experience:

  • Experience in a similar role, ideally in disability services, social work, or a community care setting.
  • Strong knowledge of the NDIS, including understanding of funding categories and service delivery requirements.
  • Excellent organisational and time-management skills, with the ability to manage multiple tasks.
  • Strong communication and interpersonal skills, with a compassionate and participant-focused approach.
  • Ability to work independently and as part of a collaborative team.
  • Tertiary qualifications in Disability Services, Social Work, or a related field.
  • Experience with case management and service planning within the NDIS framework.
  • Demonstrated ability to resolve complex issues and maintain positive stakeholder relationships.
