About the role:
Working out of our office in Northcote this role will see you assist with our intake of home care services. Utilising your astute customer centric skillset, you will work closely with Community Team Leaders and Care Advisors to provide high quality care services to our clients through the Northern Metro region.
You will be the key contact for clients and play an active role in supporting our Care workers maintaining day-to-day service delivery and coordination of client services including: scheduling, intake, records management, monthly reporting and document control.
This is a Fixed term part time role with the possibility to extend working 22.5 hours 3 days a week.
Tasks include:
- Develop and maintain relationships with clients and/or their carers to ensure care needs are being provided
- Service plan development and ongoing review
- Initial engagement with perspective clients
- Annual review of existing clients
- Monitor and manage group email inboxes, incoming calls and walk in clients and community members
- Ensure that client finance information is managed in a timely and accurate manner
- Ensure that the administrative requirements of the program are maintained in accordance with Departmental and organisational requirements
About you:

- Great customer service skills which are friendly and professional and are known for your caring and compassionate nature
- Experience in Commonwealth Supported Home Care Pacakages
- Previous administrative and rostering/scheduling experience in community services (CHSP/HCP)
- Experienced with invoice reconciliation
- Experience with accounts software and CRM systems
- Ability to manage multiple requests in a fast-paced environment
- Experience within the aged care sector
- Thrive in a team and also appreciate the one to one interactions

Essentials for working in Aged Care:
- A current Working With Children Check card
- Current and ongoing flu vaccinations
- Australian working rights
- Ability to pass a national police clearance and NDIS Clearance
