About First Peoples’ Health and Wellbeing
First Peoples’ Health and Wellbeing (FPHW) is a dynamic Aboriginal Community Controlled Health Service aiming to improve access to affordable primary health care in urban Melbourne. FPHW has two clinics, one based in Thomastown and the other based in Frankston. Both clinics offer fully bulk billed primary healthcare that is culturally safe, and trauma informed for the Aboriginal and Torres Strait Islander community and their families.

From November 2024, First Peoples’ Health and Wellbeing will commence services under the Commonwealth Home Support Programme (CHSP) to assist eligible Aboriginal and/or Torres Strait Islander peoples to remain living at home and in their community for longer.

The delivery of the services will be achieved through culturally safe, strength-based entry level home supports, tailored to individual’s needs in the areas of:

  • Allied Health and Therapy Services
  • Nursing
  • Specialised Support Services
  • Personal Care
  • Social Support
  • Meals
  • Transport
  • Home Maintenance and
  • Home Modifications

Services will be delivered by a multidisciplinary team of healthcare, social and allied health professionals, experienced in delivering strengths-based and trauma-informed programs and services. Together they will ensure the service delivery will align with the CHSP Program principles being choice, flexibility, consumer rights and participation.

About the role
This role is integral to delivering compassionate, responsive, and high-quality care while driving positive outcomes for consumers. We are looking for someone who is passionate about assisting older people and wants to make a meaningful difference.
Your focus will be providing outstanding person-centred care support to our clients, by building meaningful and genuine relationships with clients and their families. You will ensure that they are receiving the care and supports they need, so they can lead their life the way they choose.
You will ensure high standards of client care; appropriate risk management and documentation is upheld whilst maintaining strong community relationships. This role requires a sound understanding of My Aged Care Australia, Commonwealth Home Support Programme (CHSP) and the Aged Quality and Safety Standards.

Responsibilities
• Manage a caseload of consumers in our CHSP program. The role will consist of end-to-end management of our clients.
• Respond to inbound calls from potential new customers about the services we provide and conduct assessments and reviews when there is a new client intake.
• Identify and manage deterioration of individuals with increased service or referral as required.
• Refer and advocate when appropriate for increased access to services within the Support at Home Program.
• Guide new clients through every step during their onboarding process and provide ongoing support.
• Ensure exceptional high standards of report writing/case notes are all up to date and any concerns regarding clients are proactively managed
• Ensure all targets, KPI’s and program deliverables are met
• Incidents (SIRS Reporting), feedback, complaints and improvements reporting to be created in an accurate and timely manner and in accordance with legislation, FPHW policies and procedures.
• Contribute to continuous quality improvement.
• Use a person-centred approach, develop individual client care plans, regularly conduct client surveys, and review individual client needs and goals, and implement initiatives and actions in response to client feedback.
• Collaborate with clients, fostering an understanding of their needs and aspirations in order to explore options and develop actions that will best meet their needs.

About you
• Demonstrated experience in building positive relationships with a broad range of internal and external stakeholders that contribute to high quality service provision for clients
• Demonstrated ability to work autonomously under limited direction.
• Must have strong understanding of My Aged Care, retrieving codes and support plans for the provision of service under CHSP.
• Highly motivated with strong organisational, interpersonal and problem-solving skills with a strong attention to detail
• Excellent communication skills – both written and verbal (essential)
• Have a compassionate approach to care and improving the quality of others’ lives.

Essential
• At least two years’ industry and case management experience within the health care or home care sector.
• An awareness and understanding of issues confronting older people in a home and/or community setting.
• Ability to apply a person-centred care approach in the assessment, planning and care coordination of clients.
• Ability to communicate effectively with and gain cooperation from staff, recipients, and First Nations residents from a diverse range of age groups and backgrounds.
• Knowledge of My Aged Care (MAC) service provider portal and experience with client file management systems

Desirable
• Tertiary qualification in Community Services, Allied Health, Human Services, or a related field, and experience in working with older people in a home or community setting; or lesser formal qualifications with substantial experience in working with older people and people within a home or community setting.
• Degree or Diploma in Community Services is not essential but would be highly regarded.
