What You'll Do:
- Deliver high-quality care services in line with organisational policies, Aged Care Quality Standards, and program guidelines
- Coordinate client intake, assessments, and care planning, ensuring each plan is tailored to meet individual needs and goals
- Monitor and adjust care plans based on changing circumstances, referring clients to Home Care Packages when necessary
- Collaborate with clients, families, and service providers to create a seamless, high-quality service experience
- Maintain accurate documentation, ensuring timely and compliant reporting
- Promote a professional, respectful workplace culture, enhancing client experience and satisfaction
What We’re Looking For:
- Demonstrated experience in a similar role, with a strong background in aged care, health, or community services (experience in a not-for-profit setting is a plus)
- Advanced communication and interpersonal skills, with a strong focus on building relationships and ensuring client satisfaction
- Understanding of the Aged Care Act, CHSP Guidelines, and Aged Care Quality Standards
- Cert IV, Diploma, or higher in Community Services or a related field (desirable)
- A passion for supporting seniors, particularly from culturally diverse backgrounds
Additional Requirements:
- WWCC and NDIS Workers Screening, or be willing to obtain
- Rights to work in Australia
- Up-to-date First Aid and CPR certificates, or be willing to obtain
- NDIS Workers Orientation Module – Quality, Safety & You
- Have a home office to work safely and effectively
