Care Manager
North Sydney | Residential Aged Care

A well-established residential aged care provider located close to North Sydney is seeking an experienced, people-focused Care Manager to join their team.

This is an excellent opportunity for a Care Manager looking to take the next step in their career within a supportive and established nursing home, backed by a strong organisational structure.

You will work closely with the Facility Manager, fellow Care Managers and RN team, with additional support from dedicated Quality and Education professionals.

If you are a clinically capable and approachable leader with at least one year of Care Manager experience, this role offers the opportunity to develop your career within a respected aged care provider.

About the Role

As a Care Manager within the home, you will provide clinical leadership to the RN and care teams, monitor standards of care and work closely with the Facility Manager to support positive resident outcomes.

The role combines clinical leadership, team development, quality and compliance, and day-to-day oversight within a supportive and collaborative environment.

Key Responsibilities

• Lead and support RNs and care staff to deliver high-quality, person-centred care
• Oversee clinical care planning, documentation and medication management
• Monitor clinical indicators, incidents and risks, implementing appropriate actions where required
• Support the team to maintain compliance with the Aged Care Quality Standards
• Build strong relationships with residents, families and key stakeholders
• Coach and mentor RNs and care staff to maintain high standards of clinical care
• Work closely with the Facility Manager and Quality and Education teams on continuous improvement
• Support audits, accreditation preparation and the implementation of policies and procedures

About You

You are a calm, approachable and clinically capable leader who genuinely enjoys supporting teams and improving outcomes for residents.

You will also bring:

• At least 1 year of experience as a Care Manager within residential aged care
• Current AHPRA registration as a Registered Nurse
• Strong understanding of clinical governance and the Aged Care Quality Standards
• Confidence managing clinical issues, incidents, complaints and audits
• A supportive leadership style with the ability to coach and develop teams
• A genuine commitment to person-centred care and resident outcomes
