About us:

Cairns and Hinterland Hospital and Health Service covers an area of 142,900 square kilometres. We go as far as Tully in the south, Cow Bay in the north and Croydon in the west.

The Adult Community Health Service provides multidisciplinary, person-centred care across a range of community-based programs and clinical services. These include Care Coordination, Community Rehabilitation, Post-Acute Care, Rapid Response Falls, the Queensland Ambulance Service Falls Co-Responder service, Pulmonary Rehabilitation, Persistent Pain Management, Refugee Health, Aboriginal and Torres Strait Islander Health Worker services.

The multidisciplinary workforce includes nursing, Aboriginal and Torres Strait Islander Health Workers, occupational therapy, physiotherapy, social work, psychology, speech pathology, dietetics and other clinical and support roles.Services are delivered across the Cairns region through a combination of clinic-based, home-based, outreach and community care models, operating from multiple service sites including Cairns North, Edmonton and Smithfield Community Health Centres and the Cairns South Health Facility.

The service works in close partnership with consumers, families and carers, acute and outpatient services, primary care providers, General Practitioners, Aboriginal and Torres Strait Islander health services, community organisations and non-government service providers. The service supports consumers to manage their health and wellbeing, improve functional independence, reduce the risk of avoidable hospital presentation or admission, and facilitate timely discharge, integrated care and continuity of care within the community.

About you:

In your new role you will......

  • Deliver high level evidenced based therapy programs to a frail elderly client case load.
  • Initiate and develop public relations and marketing for the Rapid Response Falls Service and Adult Community Health Service throughout the CHHHS region and to other stakeholders to ensure established and enhanced linkages between services supporting older people in the community.
  • Provide leadership and facilitate the development of the Rapid Response Falls Service and Occupational Therapy/Physiotherapy quality activities, clinical practices, procedures and protocols as requested by the Team Leader. Monitor and report on clinical practices and outcomes within clinical service area and actively participate in initiating, developing and implementing quality and service improvement activities.
  • Provide day to day operational supervision and support to Allied Health Assistants, other Occupational Therapists/Physiotherapists and students as required.
