Job Description

As an Aged Care Comprehensive Assessor, you'll play a crucial role in identifying the needs of older adults and connecting them with services that help them live independently with the highest quality of life across the Townsville (Northern) Region.

Daily travel is a core requirement of this role. You will be required to travel throughout your service area every day to meet business and client needs, providing support where it’s needed most

Your Role:

  • High-Quality Assessments: Conduct thorough Aged Care Assessments to identify the needs of older adults.
  • Service Referrals: Facilitate referrals to essential services that support independent living.
  • Promote Functionality: Actively promote the retention and maximization of function and capability in daily living.
  • Engage Stakeholders: Work closely with families and carers, recognizing them as key stakeholders.
  • Compliance: Ensure all assessments are conducted in line with Department of Health contractual requirements.

Must haves:

  • Tertiary education in a health-related discipline (e.g., nursing, occupational therapy, physiotherapy, social work).
  • Current unrestricted General Registration with AHPRA or relevant professional association.
  • At least 1 year in aged care, including client assessments and/or service delivery.
  • Current Australian driver’s licence, comprehensively insured vehicle, and the ability to travel within your service area.
  • Proficiency in Word, Excel, Outlook, and client management systems.

Aged care and disability assessments

APM Assessment Services provide assessments for older adults to assist people to live independently at home and be supported in residential care. We also carry out functional assessments to assist people living with disability to access the NDIA and support employment.

My Aged Care program

APM provides assessment services to the Australian Government’s My Aged Care home support program. Many older people living at home need support to stay living independently and to access the right support services. Our assessors visit people in their home to determine the level of support they need and refer them to services in their local community.

Disability

APM is part of the National Panel of Assessors which helps determine the support needs for people with disability in the workplace. And on the panel for Independent Assessments undertaken for the NDIA. As part of the APM team, you’ll be joining an exceptional group of qualified, friendly and experienced allied health professionals and rehabilitation counsellors, who perform the assessments.
