We are seeking an experienced and motivated Social Worker to join the Veteran Wellbeing Support Coordination team at Club Active for our Brisbane clinics.

This is a unique opportunity to work within an innovative allied health environment supporting veterans through psychosocial intervention, wellbeing support, advocacy, and case coordination alongside Exercise Physiologists, Physiotherapists and Dieticians.

The successful applicant will be confident working autonomously, making independent clinical decisions, and managing a varied caseload of veterans presenting with complex psychosocial needs.

About the Role

This position offers flexibility, autonomy, and the opportunity to help shape and grow social work services within the Brisbane region, while being supported by the established Gold Coast Veteran Wellbeing team.

You will:

Provide psychosocial assessments and therapeutic support to veterans

Deliver case management, advocacy, and referral coordination

Liaise with GPs, specialists, DVA providers, families, and community supports

Support veterans experiencing, adjustment difficulties, social isolation, chronic health conditions, grief/loss, identity transition, and complex psychosocial stressors

Facilitate wellbeing-focused engagement and group initiatives

Work collaboratively within a multidisciplinary allied health environment

Maintain accurate clinical documentation and professional communication

About You

We are looking for someone who:

Has a minimum of 2 years' experience in social work, mental health, community health, veteran support, or related fields

Is confident working independently and exercising sound clinical judgement

Demonstrates strong interpersonal and communication skills

Can effectively manage presentations and competing priorities

Is warm, adaptable, and passionate about veteran wellbeing

Values flexibility, initiative, and relationship-based practice

Enjoys building programs and services within a growing space

Essential Requirements

Bachelor Degree in Social Work

Membership with the Australian Association of Social Workers

Current driver's licence and reliable vehicle

National Police Check

Working with Children Check (or ability to obtain)

Australian citizenship is required for this position

Desirable

Experience working with veterans, mental health, or chronic health conditions

Knowledge of DVA systems and community supports for Veterans
