Key Responsibilities:
Conduct pre-admission screenings and develop individualized care plans for patients and caregivers.
Coordinate with nurses and clinicians to provide holistic home care, monitor patient progress, and report any care gaps or changes in condition.
Manage patient and caregiver enquiries, provide care plan updates, and maintain accurate documentation of care and treatment progress.
Participate in interdisciplinary team meetings and collaborate with community resources to ensure continuity of care and appropriate referrals.
Monitor patients' vital signs through telehealth systems, escalate abnormalities promptly, and ensure compliance with telehealth monitoring protocols.
Deliver basic patient education and health coaching on lifestyle changes, including diet, nutrition, and exercise.
Support operational excellence by coordinating patient care equipment and supplies, participating in quality improvement and risk management initiatives, and carrying out other assigned duties as needed.
Requirements:
Diploma in Social Work / Health Services Management or equivalent
Strong interpersonal, communication, and counseling skills
Empathy, patience, and the ability to work with people from diverse backgrounds
Familiarity with local community resources and social services
