We are seeking an exceptional Care Coordinator who has a passion for helping others, a strong commitment to client care and the ability to work collaboratively with a team of healthcare professionals to deliver the best possible outcomes.
Key Responsibilities:
As our Care Coordinator, you will:
- Understand the Commonwealth Home Support program and HACC programs
- Conduct assessments: including identifying any health risks, medical conditions, or social and environmental factors
- Develop care plans
- Coordinate care
- Provide support and education.
What we're looking for:
- Current Australian driver's licence
- National police check
- Working with vulnerable people/children check
- Minimum Certificate III in a relevant discipline
