Nurse Next Door Melbourne Outer East is seeking an exceptional Clinical Care Coordinator and Team Leader to lead a growing team of nurses and caregivers while delivering life-changing outcomes for older Australians and people living with disability. We call this central leadership role a Care Designer.

Your Next Adventure (The Role)
Reporting directly to our General Manager, you will lead a dynamic team to drive clinical excellence, safety, and happiness for our clients in their own homes.
You will lead:
A Growing Multidisciplinary Care Team: Assist in the performance management, mentoring, and development of a passionate team of caregivers and nurses.
Client Experience Improvements: Consult with potential clients on premium home care options and design tailored, meaningful care plans that foster deep relationships.
Clinical Governance, Quality & Compliance: Oversee client clinical care, maintain meticulous documentation records, and champion the high clinical standards set by Nurse Next Door.
Mission-Driven Care: Embody our core philosophies of Making Lives Better® and Happier Ageing®, enabling our clients to live fulfilling lives at home doing what they love.
Hands-On Connection: Maintain a hands-on connection with client care and provide clinical support where required to ensure continuity of service and exceptional client outcomes (5 days per week with a rotating, occasional evening/weekend on-call roster).

What You Bring (Requirements)
Minimum of 5 years of experience as a Clinical Lead, Manager, or Team Leader within a community care environment.
Minimum of 10 years of experience as a Registered Nurse.
Current AHPRA Registration as a Registered Nurse.
Proven previous clinical leadership experience across NDIS and Aged Care sectors.
Solid, current clinical experience in wound care, catheterisation, medication management, diabetes management, and manual handling practices.
Impeccable organisation, time management, and verbal/written communication skills.
High proficiency in using mobile apps, G-Suite, and various software platforms.
Mandatory Compliance: NDIS Worker Screener (or willingness to obtain), Working With Children Check (Employee), and an up-to-date CPR & First Aid Certificate.
