We are searching for an experienced and knowledgeable Case Manager to join our Bethlehem House Team. Bethlehem House has been operational for over 50 years, offering accommodation and support for up to 70 men experiencing or at risk of homelessness in Hobart.

The Case Manager is a key member of the Bethlehem House Case Management Team, providing person-centred case management and support to residents throughout their journey at Bethlehem House. Reporting to the Executive Manager, this role additionally works collaboratively with the Support Worker Team to ensure residents receive coordinated, holistic support that promotes stability, wellbeing, and positive outcomes.

The Case Manager is responsible for receiving and triaging referrals, conducting intake assessments, and managing residents throughout their residency. Using a strengths-based approach, the Case Manager develops individualised case plans, establishes meaningful goals with residents, and provides ongoing support, advocacy, and coordination to assist them in achieving sustainable outcomes.

The role also develops and maintains effective partnerships with external agencies and community services, creating strong referral pathways that enable residents to access the services and supports they need.

Success in this role requires a collaborative approach, excellent interpersonal and communication skills, sound professional judgement, and a commitment to reflective practice and continuous improvement. The Case Manager contributes to a positive team culture while upholding the mission, values, and ethos of St Vincent de Paul Society Tasmania.

Essential Requirements

Relevant demonstrated experience in community service, mental health support, drug and alcohol program support and/or a social services setting is highly advantageous.

Understanding of the current housing crisis, the experience of homelessness, and the barriers faced in overcoming associated challenges.

Experience in providing person-centred case management, whilst working towards achievable goals.

Eagerness to create and manage appropriate stakeholder relationships with relevant organisations to provide a wider range of services and support to residents.

Ability to work as part of a cohesive team, taking a supportive approach to engaging with colleagues and residents that embraces trauma-informed care and support.

Strong understanding of professional boundaries and the ability and willingness to consistently maintain them.

High-level administration capacity, and the ability to adapt to and learn new platforms quickly.

Knowledge and understanding of risk management principles and their application to this environment.

Strong written communication skills, with the ability to produce accurate, concise, subjective and detailed case notes and reports.

Exceptional time management skills and the ability to manage competing priorities.

Ability to communicate with a wide range of cohorts, displaying empathy and fairness in all interactions.

Strong commitment to confidentiality and an understanding of privacy principles.

Commitment to work together with the greater SVDP Tas team in achieving against strategic and operational plans.

Demonstrated ability to adapt to changes in work priorities, styles and environment, recognising the need to be responsive in a context that can change rapidly.
